Open Enrollment is now closed. Submitted changes are effective January 1, 2027. Processing is completed in December.
Once processed, Medical cards and HMO Dental cards may be expected in the mail at home.
For PPO or EPO Medical and PPO Dental plan, dependents may use the subscriber’s card.
- Medical Cards: Sydney Health App.
- PPO Dental Cards: Access Digital Cards (no cards are mailed)
FSA subscribers may expect a confirmation email with portal access instructions from Igoe Administrative Services by December. Newly enrolled participants may also expect a Point of Sale/Service card in the mail. Those who submitted enrollment to continue contributions into 2027 should retain the card; unexpired cards are reloaded.

PPO subscribers only: When a non-contracted provider has been paid for services, a super bill (detailed, paid receipt) should be submitted through the Sydney Health App. Allow up to 8 weeks to receive an Explanation of Benefits and any reimbursement due you.

- Then, select DHMO Concordia Plus General Dentist.
If your dentist is not a part of our network, you can nominate him or her for participation. To nominate your dentist, visit the Members section, select Forms, and click Nominate Your Dentist.
